Job summary

Well Care Health in the Carolinas seeks a compassionate Hospice Registered Nurse Case Manager to deliver end-of-life care and coordinate with the hospice team to ensure comfort and dignity for patients and families.

You will assess needs, develop care plans, and educate patients and families about hospice processes, while supporting emotional and spiritual well-being. An RN license in NC and CPR are required, with hospice experience preferred.

Qualifications

• Graduate of NLN accredited school of nursing; current RN license in NC; BSN preferred.
• Must have NC RN license and a valid driver’s license with insurance; CPR certification required.
• Minimum of one year clinical experience; hospice experience preferred.
• Working knowledge of Word, Email; basic data entry and home health billing knowledge.
• Ability to work independently and as part of an interdisciplinary team.
• Physical demands include lifting, standing, walking; some on-call work.

Responsibilities

• Complete initial, comprehensive, and ongoing assessment of patient and family needs with direct or supervised nursing services.
• Develop individualized care plans with the hospice team, incorporating palliative nursing interventions.
• Communicate with Attending and Hospice physicians about needs and treatment responses.
• Collaborate with IDG and family to provide holistic services including bereavement support.
• Demonstrate positive interpersonal relations and uphold agency mission and values.
• Contribute to high-quality care delivery daily.
