Qualifications

• RN licensure in NC is required.
• Bachelor's degree in nursing preferred.
• Experience in hospice or end-of-life care is desirable.

Responsibilities

• Assess patients and families to develop individualized care plans.
• Coordinate with hospice interdisciplinary team for plan of care.
• Educate patients and families about hospice processes and care.
• Provide direct nursing services and support to patients.
