Job summary

Well Care Health is seeking a compassionate Hospice Registered Nurse Case Manager to provide skilled nursing care for hospice patients and to develop personalized care plans. You will educate families on end-of-life care, coordinate with the hospice team, and ensure seamless interdisciplinary collaboration to uphold high-quality patient care.

This role requires an NC RN license, a relevant bachelor's degree, and experience in hospice or palliative care.

Qualifications

• Graduation from an NLN-accredited nursing program.
• Current RN license in North Carolina (or state of practice as required).
• Bachelor’s degree in nursing or related field preferred.
• Basic knowledge of home health care billing and medical terminology.

Responsibilities

• Provide skilled nursing care to hospice patients focusing on pain management and comfort.
• Develop individualized care plans with the interdisciplinary team.
• Educate patients and families about hospice processes and end-of-life care.
• Communicate with physicians about patient status and orders.
• Coordinate with the hospice team to ensure comprehensive, compassionate care.
