Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for Haywood Hospice, part of LHC Group within the Optum family. You will oversee the Plan of Care for hospice patients, collaborating with IDG, physicians and families to promote physical, emotional and spiritual well-being while delivering high-quality care.

The role requires an active RN license in the state, a valid driver's license, and the ability to travel to patient homes.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members to provide care daily.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding the patient's condition.
• Provide care in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
