Job summary

Corewell Health is seeking a Hospice Nurse Case Manager to coordinate end-of-life care for patients and their families. You will assess patient needs, develop individualized care plans, and collaborate with an interdisciplinary team to ensure quality hospice services.

The role involves traveling to patients’ homes, nursing facilities, and hospitals, providing compassionate care, education, and ongoing support while maintaining compliant documentation and safety standards.

Qualifications

• Associate's degree in nursing.
• 2 years of relevant experience.
• Michigan RN license upon hire.
• State of Michigan driver's license upon hire.
• Bachelor's degree preferred.
• 1 year of relevant experience (preferred).

Responsibilities

• Assess patient needs and develop individualized care plans.
• Coordinate care with interdisciplinary team for hospice patients.
• Administer medications and manage symptoms.
• Educate patients and families on disease progression and care options.
• Travel to patients' homes, nursing facilities, and hospitals to provide direct care.
• Maintain accurate documentation and ensure regulatory compliance.
