Job summary

Phoenix Home Care and Hospice is seeking a full-time Hospice RN Case Manager to support patients in Wichita, KS and surrounding areas. The role involves coordinating care with an interdisciplinary team, educating families, and guiding care decisions to enhance quality of life.

Salary range is $75,000–$90,000 annually, with benefits and training provided. The ideal candidate brings energy, motivation, and strong organizational and interpersonal skills, along with a valid Kansas or Compact RN

Qualifications

• Kansas or Compact RN License is required.
• Valid driver’s license and auto liability insurance is required.

Responsibilities

• Provide education to patients and family members regarding the process and plan of care.
• Consult with the attending physician and Hospice Medical Director regarding changes in condition and plan of care.
• Coordinate hospice care with team members and patients/family to meet needs.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and informational visits.
