Job summary

Ilocatum in Dallas, TX is seeking a Hospice Registered Nurse Case Manager to coordinate and oversee patient care in the Dallas area south of Downtown, including Cedar Hill, DeSoto, Red Oak, and Duncanville. This full-time role focuses on comprehensive assessment, symptom management, and care planning with the interdisciplinary team.

The RN will educate patients and families, document thoroughly, and ensure high-quality end-of-life care while working independently and making sound clinical

Qualifications

• Two years of RN experience in a clinical care setting.
• Current Registered Nurse license valid for practice in Missouri.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients’ hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
