Job summary

ExcelliSearch, Inc. is seeking a dedicated Hospice RN Case Manager for the Dallas area south of Downtown, including Cedar Hill, DeSoto, Red Oak, and Duncanville.

This full-time role coordinates patient care, develops plans of care, conducts clinical assessments, and collaborates with an interdisciplinary team to support patients and families. The ideal candidate holds an active RN license, CPR certification, and at least two years of RN experience in a clinical setting, with hospice experience

Qualifications

• Active Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team
• Complete all required documentation accurately and on time
