Ashton North LLC is seeking a Hospice RN Case Manager to coordinate and oversee patient care in the Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville. This full-time role focuses on developing plans of care with the interdisciplinary team and providing symptom management.

The position requires an active RN license, CPR certification, and strong decision-making and communication skills to educate families and coordinate care effectively.

Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience preferred
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Responsabilités

• Coordinate and oversee all aspects of assigned patients' hospice care
• Develop and implement individualized plans of care with the interdisciplinary team
• Perform head-to-toe assessments, monitor symptoms, and manage medications
• Educate patients and families regarding condition, needs, and care
• Communicate patient progress with clinical leadership, physicians, and the care team
• Complete all required documentation accurately and on time
