Job summary

RON SUNSHINE ASSOCIATES LLC seeks a Hospice RN Case Manager for the Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak and Duncanville. This full-time role coordinates patient care and implements individualized plans of care with the interdisciplinary team.

You will perform clinical assessments, symptom management, and educate patients and families about conditions and needs. You will ensure timely documentation and communicate patient progress to leadership and the care

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current CPR certification.
• Two years RN experience in a clinical care setting.
• One year hospice experience preferred.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.

Skills

Current Registered Nurse license

Current driver's license

Current CPR certification

Two years RN clinical experience

Hospice experience (1 year) preferred

Reliable transportation & auto/liab

Independent work ability

Strong written/verbal/interpersonal
