Zaddy Solutions seeks a Hospice RN Case Manager for the Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville. This full-time position coordinates and oversees patient care, develops plans of care with the interdisciplinary team, and ensures compassionate, evidence-based hospice support.

The role requires a Missouri RN license, CPR, a valid driver's license, and hospice experience.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsabilités

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.
