Job summary

Community Healthcare of Texas is seeking an RN Case Manager in Waco, TX to coordinate admission and plan of care for hospice patients. The role involves direct nursing care, developing patient-specific care plans, medication administration, and collaboration with physicians and interdisciplinary teams.

The ideal candidate has Texas RN licensure, 1 year of nursing experience, hospice case management experience (2+ years preferred), and strong computer skills.

Qualifications

• Registered Nurse in Texas.
• Hold a valid driver’s license and have a dependable means of transportation.
• 1 year of nursing experience.
• Case Management experience (hospice).
• 2+ years of experience in hospice case management strongly preferred.
• Proficient skills using a computer; Microsoft Office suite experience is preferred.

Responsibilities

• Provide professional nursing care by utilizing all elements of the nursing process.
• Assess and evaluate the patient’s status by writing and initiating a plan of care; regularly re-evaluating patient and family/caregiver needs.
• Develop a care plan with goals based on nursing diagnosis and hospice actions; involve patient and family.
• Administer medications and treatments as prescribed by the physician.
• Provide healthcare instruction to the patient as appropriate per assessment and plan.
• Coordinate patient care as Case Manager when assigned by the clinical Supervisor.
• Maintain accurate clinical notes regarding patient condition and care.
• Communicate with physician regarding patient needs and changes in condition; obtain orders as needed.
• Coordinate with community health-related persons to implement the care plan.
• Maintain a safe environment for the patient.
• Collaborate with family/caregiver and hospice team.
• Attend interdisciplinary meetings.
• Participate in on-call rotation; may assist with admits.
• Manage assigned patients equitably regardless of severity.
• Supervise LVN and CNA care plans.
