Community Healthcare of Texas is seeking a Field RN to coordinate admission and ongoing plan of care for hospice patients in the Waco, TX area. This PRN field position requires strong nursing skills and a compassionate approach to supporting patients and families.

The role emphasizes assessing conditions, updating plans of care, documentation, and collaboration with Case Managers to ensure high-quality end-of-life care. A Texas RN license and hospice experience are highly valued.

Formación

• Registered Nurse in the State of Texas.
• Hold a valid driver's license and have a dependable means of transportation.
• 2 years of nursing experience required.
• 2+ years of hospice experience strongly preferred.
• Proficient skill using a computer; Microsoft Office Suite experience preferred.

Responsabilidades

• Assess patient's condition and revise the Plan of Care.
• Explain hospice services to patients and families.
• Coordinate admission of patients and ongoing plan of care.
• Complete daily paperwork and charting per Policy.
• Communicate significant findings with Case Manager.
