Job summary

Community Healthcare of Texas is seeking a Field RN in Waco, TX to provide hospice and palliative care services. The role involves meeting with patients and families, coordinating admissions, and managing ongoing plans of care on a PRN basis.

The position requires Texas RN licensure, a valid driver's license, and hospice experience is preferred. Strong computer skills and the ability to document accurately are essential.

Qualifications

• Registered Nurse in the State of Texas.
• Hold a valid driver's license and have a dependable means of transportation.
• 2 years of nursing experience required.
• 2+ years of hospice experience preferred.
• Proficient with computers; Microsoft Office experience preferred.

Responsibilities

• Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.
• Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
• Provide emotional support to patients and families while instructing them in activities of daily living.
• Complete daily paperwork and charting, thoroughly and accurately, according to Policy.
• Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.
