Job summary

Redcrest Hospice in Lafayette, IN is seeking a Registered Nurse Case Manager to guide patients and families through end-of-life care. You will work with an interdisciplinary team to provide compassionate symptom management, develop a Plan of Care, and ensure smooth admissions, transfers, and discharges.

The role requires an active RN license in Indiana, 1 year acute care experience preferred, hospice experience desirable, and a valid driver's license.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting is preferred.
• Hospice experience is desired.
• Valid driver's license and auto insurance in your name as a driver.

Responsibilities

• Symptom Management: assess and collaborate with physician and Medical Director to manage patient symptoms.
• Plan of Care: evaluate and individualize the plan of care, updating as needs change.
• Continuity of Care: manage admissions, transfers, and discharges with the interdisciplinary team.
• Family Educator/Advocate: counsel patient and family on disease progression and care planning.
• Documentation: complete all clinical documentation per agency protocol and Medicare guidelines.
• Operations: oversee total care of patient provided by nurse aides and LPNs; attend trainings.
