St. Croix Hospice is seeking a Registered Nurse Case Manager to provide in-home, patient-centered care in the Topeka/Wichita area. You will develop personalized plans of care, coordinate with a care team, and support patients and families through the end-of-life journey.

The role emphasizes compassionate symptom management, autonomous practice, and collaboration with physicians, social workers, and aides. A valid RN license and hospice experience are required.

Qualifikationen

• Active RN license in good standing.
• 1+ year acute care experience; hospice/home health preferred.
• Ability to assess patient needs and develop care plans.
