Job summary

St. Croix Hospice is seeking a Registered Nurse in Topeka, KS to provide day-to-day hospice patient care under the direction of the RN Case Manager. The role emphasizes comfort, quality of life, and family support in home settings.

The RN administers medications, maintains patient records, and collaborates with an interdisciplinary team to update the Plan of Care and ensure patient dignity and choice at the end of life.

Qualifications

• Graduate of an accredited school of nursing
• Current RN license in state(s) of practice
• Ability to pass background and drug test
• Valid driver’s license and reliable vehicle

Responsibilities

• Provides day-to-day hospice patient care under the Plan of Care and RN supervision.
• Administers medications and treatments as prescribed and documents care.
• Completes comprehensive patient and family assessments and updates Plan of Care accordingly.
• Maintains accurate clinical notes and pain/symptom management records.
• Communicates with physicians and interdisciplinary team about patient needs and changes in condition.
• Coordinates Plan of Care with community health partners and ensures equipment and services are available.
