Phoenix Home Care is seeking a Hospice RN Case Manager to provide compassionate nursing care to clients with terminal diagnoses, collaborating with the interdisciplinary team to plan patient- and family-centered care. This full-time role serves Kansas City, MO and surrounding areas, offers a pay range of $75,000–$90,000, and includes benefits, mileage reimbursement, and 401(k).

Qualifications

• Missouri RN license required (or compact state license).
• Valid driver’s license and auto liability insurance.
• Excellent communication and teamwork with interdisciplinary care teams.

Responsibilities

• Provide education to patients and family members regarding the care process and guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director regarding changes in patient condition and care plans.
• Coordinate hospice care by collaborating with other team members and with patients/families to meet needs.
