Phoenix Home Care and Hospice in Kansas City, MO is seeking a skilled RN Case Manager to deliver compassionate hospice care to clients at home and with their families. You will work with the interdisciplinary team to develop and implement plans of care that reflect patient goals and comfort, while guiding families through difficult healthcare decisions.

Qualifications

• Active Kansas or Compact RN license.
• Hospice experience preferred; training provided.
• Valid driver's license and auto insurance.

• Pay Range: $80,000 - $90,000
• $5000 Sign On Bonus

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.
