Phoenix Home Care & Hospice in Topeka, KS is seeking an RN Case Manager to deliver compassionate nursing care to clients with terminal diagnoses, coordinating with an interdisciplinary team to align care plans with patient and family goals.

The role requires energy, strong organizational and interpersonal skills, and the ability to guide healthcare decisions while supporting professional growth. This full-time position is offered with a pay range of $75,000–$90,000 and weekday daytime hours.

Qualifications

• Active Kansas or Compact RN license.
• Valid driver’s license and auto liability insurance.
• Hospice experience preferred.

Responsibilities

• Provide nursing care, education, and support to patients and families.
• Complete initial and ongoing assessments.
• Coordinate care with physicians and the hospice interdisciplinary team.
• Develop and update patient-centered plans of care.
• Provide guidance with healthcare decisions.

• Pay Range: $75,000-$90,000 | Full-Time | Monday-Friday, Days
• Topeka, KS and surrounding areas

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.
