Job summary
Hospice of North Alabama is seeking a dedicated RN Case Manager to join our team in Cullman, AL. This role covers Morgan County and parts of Lawrence County, delivering compassionate hospice care from a patient-centered perspective.
The RN will assess patient needs, develop care plans, administer treatments, manage pain, and support families while coordinating with an interdisciplinary team. A valid driver's license and nursing license are required.
Qualifications
- Graduate of an accredited nursing school with current state licensure
- One year nursing experience required, 3+ years preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Must have a valid driver’s license and reliable vehicle
Responsibilities
- Assess and monitor physical, emotional, and psychological needs of patients
- Create hospice care plans aligned with patient goals
- Direct nursing care: administer medications and treatments
- Provide pain and symptom management
- Educate and support patients’ families and caregivers
- Collaborate with interdisciplinary team and maintain documentation
- Participate in on-call rotation as required by the branch
