Job summary

Hospice of North Alabama in Decatur, AL is seeking a compassionate Registered Nurse to join our hospice team as an RN Case Manager. The role provides patient-centered care in a home-like setting, coordinating plans and delivering skilled nursing services in the community.

Responsibilities include assessing needs, creating care plans, administering medications, managing pain, and guiding families. We offer comprehensive benefits and a supportive environment with opportunities for growth.

Qualifications

• Graduate of an accredited nursing school with current licensure.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must have a valid driver's license and insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients' wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team; maintain timely documentation.
