Hospice of North Alabama is seeking a dedicated RN Case Manager to coordinate patient-centered hospice care in the Cullman, AL area. The role involves assessing patients, creating personalized care plans, administering nursing care, and collaborating with a dedicated interdisciplinary team to ensure comfort and quality of life.

On-call rotation is required. Ideal candidates will have an accredited nursing degree, state licensure, and a commitment to documentation excellence, with 1 year of

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile insured and in good working order.

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients' wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
