Job summary

Bristol Hospice Jackson, MS – Flowood, MS is seeking a registered nurse Case Manager to provide compassionate, patient- and family-centered end-of-life care. You will develop individualized care plans, monitor vitals, administer physician-ordered treatments, and collaborate with interdisciplinary teams in a day-shift role.

Qualifications

• Must hold a valid RN license in good standing.
• Strong communication and observation skills.
• Able to collaborate with interdisciplinary teams.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families to ensure comfort and dignity.
• Monitor vital signs and document changes.
• Administer physician-ordered medications and treatments.
• Educate families on care techniques and symptom management.
• Communicate patient changes promptly for timely action.
• Maintain up-to-date clinical notes reflecting progress.
• Collaborate with care team to ensure comprehensive care.
