Caring Hospice Services is seeking a Registered Nurse Case Manager in Pennsylvania to perform ongoing patient assessments, maintain thorough notes, and coordinate care. You will liaise with patients' families and caregivers to ensure compassionate, effective hospice services.

Requirements include PA RN license, a nursing degree (AS/BS/MSN), at least 2 years in Med-Surg, Home Health, or Hospice, and the ability to lift up to 25 lbs. Bilingual skills in Spanish or Mandarin are a plus.

Competenze

• Must hold active PA RN license.
• Associate/Bachelor's/Masters Degree in nursing.
• Minimum 2 years Med-Surg., Home Care, or Hospice experience.
• Ability to lift up to 25 lbs.
• Bilingual skills in Spanish or Mandarin are a plus.

Mansioni

• Perform ongoing patient assessments and maintain diligent notes and records.
• Maintain constant contact with patient families and caregivers.
• Provide nursing care with emphasis on pain and symptom management.
