Job summary

Caring Hospice Services is seeking a compassionate Hospice Registered Nurse Case Manager to perform ongoing patient assessments and maintain detailed notes documenting the care plan effectiveness. You will stay in regular contact with patient families and caregivers, ensuring quality, patient-centered care throughout the hospice journey.

Responsibilities include managing pain and other symptoms, supervising Home Health Aides, and connecting families with community resources.

Qualifications

• Licensed as a Registered Nurse in Pennsylvania.
• Associate/Bachelor’s/Master’s Degree in nursing.
• Minimum 2 years Med-Surg., Home Care, and/or Hospice experience.
• Ability to occasionally lift up to 25 lbs.
• Bilingual in Spanish or Mandarin is a plus.

Responsibilities

• Perform ongoing patient assessments and maintain thorough notes about each patient’s care plan.
• Maintain contact with patient family members and alternative caregivers.
• Provide nursing care with emphasis on pain and other management.
• Supervise Home Health Aides.
• Refer patients’ families to applicable community resources.
• Maintain strict patient confidentiality.
• Provide patient care directives to family members and caregivers.
