Job summary

Geisinger Hospice-Lewistown is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients, coordinating with the IDG and physicians to ensure compassionate, high-quality care.

You will conduct initial evaluations within 48 hours of referral, develop plans of care, document progress, and support patients and families while navigating end-of-life needs. This role requires an active RN license in PA.

Qualifications

• Current and unrestricted RN licensure in the state of practice (PA).
• Valid driver’s license, own vehicle or reliable transportation.
• Current CPR certification or able to obtain within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions and patient/family responses.
• Collaborate with patient/family, attending physician and IDG for daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
• Provide care in diverse home settings irrespective of age, race, creed or income.
