Bristol Hospice Cincinnati is seeking a Registered Nurse Case Manager to provide compassionate, whole-person care and advocate for patients and families in Cincinnati, OH. This on-site role offers a daytime schedule with a competitive annual salary.

As part of a mission-driven team, you will develop care plans, monitor patient conditions, and educate families while collaborating across disciplines to ensure high-quality end-of-life care.

Qualifications

• RN license in good standing in the work state.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and implement individualized care plans.
• Deliver one-on-one support to patients and families.
• Monitor vital signs and document findings.
• Administer physician-ordered medications and treatments.
• Educate patients and families on care techniques and symptom management.
