Job summary

Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to deliver hands-on hospice nursing care and coordinate services within the South Boston, Braintree, Dorchester, Quincy, Randolph, Milton and Weymouth territory.

You will work with the interdisciplinary team to assess, plan, and implement patient-centered care, educate families, and contribute to maintaining high standards of clinical excellence and compassionate connection.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Computer proficiency including EMR; CPR certification as required by state.

Responsibilities

• Deliver hands-on hospice nursing care.
• Coordinate services and guide patients and families through the hospice journey.
• Collaborate with Clinical Director to assess client eligibility and needs.
