Job summary

UnitedHealth Group is hiring an RN Case Manager to coordinate compassionate hospice care for patients in Covington, Lexington, and Eagle Rock areas. You will assess patients, partner with the interdisciplinary team, and develop care plans that meet individual needs.

The role requires an active RN license in the state, CPR certification, and at least 1 year of nursing experience, plus a valid driver’s license and reliable transportation.

Qualifications

• Current RN license issued by the state(s) you will work in.
• Current CPR certification.
• At least 1 year of nursing experience.
• Valid driver’s license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records accurately and timely.
• Records pain/symptom management changes/outcomes.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Communicates regularly with the clinical manager and care team.
• Performs on-call responsibilities as assigned.
