CrossMed Healthcare Staffing is sourcing a Hospice Registered Nurse for client operations in North Carolina. The RN will provide skilled nursing care at patients' homes, assess needs, adjust plans of care, and support families through end‑of‑life processes.

The role requires an active RN license, CPR certification, and a minimum of one year of hospice or related nursing experience. Travel to patient residences is expected, with comprehensive benefits and on‑call support.

Formación

• RN required and licensed to practice in the state of employment.
• Minimum of 1 year experience as an RN in hospice, palliative care, home health, or related setting preferred.
• CPR certification required; valid driver's license and auto insurance may be needed.

Responsabilidades

• Perform comprehensive patient assessments and re‑assessments in patient homes or facilities.
• Deliver and document skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with an interdisciplinary team and physicians.
• Educate patients and caregivers on disease, self‑care, and end‑of‑life care.
• Communicate changes in patient condition to the care team promptly.
• Maintain accurate clinical records and ensure infection control standards.
• Participate in on‑call and family support activities as applicable.
