Job summary

Well Care Health is seeking a compassionate Hospice Registered Nurse Case Manager in North Carolina. You will manage a caseload, develop personalized care plans, and coordinate with the hospice team to deliver comprehensive end-of-life care with empathy and skilled nursing support.

This role requires an NC RN license, CPR certification, and 1 year of clinical experience (hospice preferred). On-site work, with on-call responsibilities and opportunities for growth.

Qualifications

• Education: NLN-accredited nursing program and valid NC RN license; Bachelor’s degree preferred.
• Licensure: current NC RN license and AHA CPR certification.
• Experience: minimum 1 year clinical experience, hospice experience preferred.
• Skills: strong communication, empathy, and ability to work in an interdisciplinary team.

Responsibilities

• Perform initial and ongoing assessments and provide direct or supervised nursing care per care plan.
• Develop individualized hospice care plans with the interdisciplinary team and family.
• Coordinate care with hospice team and physicians; adjust plans as patient needs change.
• Educate patients and families on hospice process, end-of-life care, and coping strategies.
• Provide emotional support to families and bereavement resources as needed.
• Demonstrate mission, vision, and values in daily work.
