St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to deliver holistic, in-home patient care.

You will develop personalized plans of care with patients and families, lead meaningful end-of-life conversations, and ensure effective symptom management with empathy and clinical excellence. Requirements include an accredited RN program (AS/BSN/MSN) and 1+ years of acute care; hospice/home health experience preferred.

Formación

• Associates, Bachelors, or Masters of an accredited nursing program (RN).
• 1+ years of recent acute care experience required; hospice/home health experience preferred.
• Develop individualized care plans with the patient and family, updating as needs evolve.
• Rotate on-call duties and ensure seamless care transitions.

Responsabilidades

• Deliver individualized, holistic support to patients in their homes.
• Lead end-of-life conversations and ensure symptom management with empathy and clinical excellence.
• Educate patients and caregivers on care techniques, medications, and diet.
• Collaborate with a care team to develop and update care plans.
