The ProHealth group of companies seeks a dedicated Hospice Registered Nurse to provide terminal care in the home setting and support families through bereavement. You will collaborate with the Hospice team to deliver compassionate, patient-centered care aligned with HIPAA policies.

Qualifications include an accredited nursing degree, active RN license in the state, and 2+ years of RN experience; hospice training is a plus and independent judgment is essential.

Qualifications

• Graduate from an accredited school of nursing.
• Licensed RN to practice in the state.
• Ability to work independently and make quick judgments.
• Ability to supervise others appropriately.
• Complete Hospice training program.
• Minimum 2 years of RN experience; preferred.
• Active patient contact within the past 3 years preferred.

Responsabilités

• Regularly assesses and reassesses Hospice patient nursing needs.
• Provides Hospice nursing services and preventive procedures.
• Initiates nursing procedures for patient care and safety.
• Observes signs and reports changes to physician/IDG.
• Teaches and counsels patient and family about care.
• Supervises and trains other nursing staff.
• Maintains patient care plans with IDG for continuity.
• Coordinates care for patients in SNF/NF/ICF or MR.
• Attends team conferences and documents records.
• Follows HIPAA and agency policies; on-call coverage as needed.
