Community Health Network is seeking a Hospice Registered Nurse to manage hospice patients, provide direct nursing care, and coordinate care across disciplines. Responsibilities include ensuring quality services as outlined by the patient’s plan of care and physician instructions.

The role primarily serves the north side of Indianapolis, including Fishers, Carmel and Westfield. Required qualifications include Indiana RN licensure, CPR certification, and at least one year of nursing experience;

Qualifications

• Graduate from an approved nursing program and hold Indiana RN licensure.
• CPR certification and valid driver's license required.
• Minimum one year of nursing experience; home care/hospice experience preferred.
• BSN preferred; graduation from an accredited program is expected.

Responsabilités

• Provide direct nursing care and case management for hospice clients.
• Coordinate and supervise other disciplines involved to ensure quality services.
• Deliver skilled care according to the patient’s plan of care and physician orders.
