Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide in-home, holistic care to patients in Pewaukee, WI. You will collaborate with a care team to create personalized care plans, lead meaningful end-of-life discussions, and ensure symptom management with empathy and clinical excellence.

The role emphasizes autonomy in practice, compassionate patient connections, and on-call responsibilities. A valid WI RN license and reliable transportation are required.

Qualifications

• 1+ years of acute care experience; hospice/home health preferred.
• Completes initial and ongoing assessments to determine hospice needs.
• Develops individualized care plans with patient and family.
• Administers medications and treatments as prescribed.
• Educates patients and caregivers on care techniques, meds, diet and symptom management.
• Rotates on-call duties and ensures seamless care transitions.
• Must have reliable transportation, valid driver’s license and auto insurance.
