Job summary

Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to provide high-quality end-of-life care to patients in their homes while supporting families with empathy and respect. You will conduct comprehensive assessments, develop and manage care plans, and coordinate with the interdisciplinary team to ensure seamless, patient-centered care.

The role requires an active RN license in Missouri, CPR/BLS certification, and the ability to work independently and with a team.

Qualifications

• RN license in the state of employment.
• CPR/BLS certification active.
• Strong interpersonal, clinical, and organizational skills.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients and coordinate care with the IDT.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management and end-of-life care.
• Monitor and update care plans based on ongoing assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in IDT meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
