Redcrest Hospice in Indianapolis is seeking a dedicated Hospice Registered Nurse Case Manager (RNCM) to guide patients and families through end-of-life care. You will work within an interdisciplinary team, ensuring symptom management and adherence to plan of care.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting is preferred.
• Hospice experience is desired.
• Valid driver’s license and auto insurance in your name as a driver
• Capable of all physical demands

Responsibilities

• Symptom management: work with attending physician and hospice Medical Director to manage patient symptoms.
• Plan of Care: evaluate and individualize the plan of care as needs change.
• Continuity of Care: oversee interdisciplinary care and admissions, transfers, discharges.
• Family Educator/Advocate: counsel patient and family on disease progression and plan of care.
• Documentation: complete clinical documentation per agency protocol and Medicare guidelines.
• Operations: supervise care provided by nurse aides and LPNs; participate in meetings.
