Emerald Coast Hospice seeks a dedicated Hospice RN / RN Case Manager to lead in-home patient care. You will perform comprehensive RN assessments, implement skilled nursing interventions, and coordinate with physicians, social workers, and other team members to build tailored care plans.

Qualifications

• RN with active license to practice in Florida.
• Hospice/palliative care experience preferred.
• Minimum 1 year RN experience in acute care, oncology, or hospice/home health.

Responsabilités

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate with interdisciplinary team to refine care plans.
• Educate patients, families, and caregivers on disease, self-care, and end-of-life care.
• Communicate changes in patient condition to care team for timely interventions.
• Maintain accurate clinical documentation and comply with infection control standards.
• Support quality improvement and scheduling initiatives.
