Heart'n Soul Hospice in Miami, FL is seeking an RN Case Manager to lead comprehensive patient assessments and coordinate end-of-life care. You will collaborate with physicians, family members and the care team to maintain high-quality, compassionate services for life-limiting illnesses.

The role emphasizes on-call support, timely documentation, and adherence to regulatory standards, while offering competitive benefits and opportunities to impact patient families meaningfully.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation and ongoing auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical setting.

Responsibilities

• Perform initial and ongoing assessments of the patient's condition and needs.
• Implement and revise the plan of care as needed.
• Coordinate care among patient, family, physicians and other professionals.
• Attend IDG meetings and participate in care planning and prioritization.
• Provide on-call coverage and timely documentation.
• Ensure compliance with laws, Medicare regulations, and policies.
