Job summary

Emerald Coast Hospice is seeking a dedicated Registered Nurse Case Manager to provide expert hospice nursing care in patients' homes and alternative settings. You will assess, plan, implement, and document individualized care while coordinating with an interdisciplinary team to ensure compassionate end-of-life support.

Qualifications

Registered Nurse licensed in Florida (or ability to obtain)
CPR Certification required
Current driver’s license and automobile insurance

Responsibilities

Perform comprehensive patient assessments and reassessments in home or facility settings
Deliver nursing interventions and symptom management per Plan of Care
Develop and update individualized care plans and visit frequency
Coordinate care with physicians, LPNs, CNAs, social workers and chaplains
Educate patients and caregivers about disease, end-of-life care and medications
Document timely clinical records and maintain infection control standards
