Job summary

Buckeye Hospice and Palliative Care Services in Hilliard, OH is seeking a Registered Nurse Hospice Case Manager to join our team. You will assess patients, develop plans of care, and coordinate hospice services to enhance quality of life for patients and families.

Responsibilities include coordinating interdisciplinary care, monitoring patient status, and educating families. Ohio RN licensed candidates with hospice or case management experience are preferred; valid driver’s license required.

Qualifications

• Current Registered Nurse license in the State of Ohio.
• Hospice, home health, or case management experience preferred.
• Strong assessment, organization, communication, and clinical decision‑making skills.
• Ability to manage multiple patients and priorities independently.
• Commitment to compassionate, patient‑centered care.

Responsibilities

• Assess patients and develop individualized plans of care.
• Coordinate and oversee delivery of hospice services.
• Monitor patient condition and communicate changes with the physician and interdisciplinary team.
• Provide skilled nursing interventions and symptom management.
• Educate patients and caregivers regarding medications, treatment plans, comfort measures, and disease progression.
• Coordinate services provided by nursing, aides, social work, chaplaincy, and other disciplines.
• Participate in interdisciplinary team meetings and care planning.
• Advocate for patient and family goals and preferences.
• Maintain accurate, timely, and compliant clinical documentation.
• Provide support and education to caregivers throughout the hospice experience.
