Buckeye Hospice and Palliative Care Services is seeking a compassionate Registered Nurse Hospice Case Manager to join our team in Hilliard, OH. You will assess patients, develop individualized plans of care, and coordinate hospice services to ensure the best possible quality of life for our patients and families in Central Ohio.

The role requires solid assessment, organizational and communication skills, with the ability to manage multiple patients and work within an interdisciplinary team.

Qualifications

• Must hold valid Ohio RN license.
• Experience with hospice, home health, or case management preferred.
• Strong assessment, organization, communication and clinical decision‑making skills.
• Ability to manage multiple patients and priorities independently.
• Commitment to compassionate, patient‑centered care.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess patients and develop individualized plans of care.
• Coordinate and oversee delivery of hospice services.
• Monitor patient condition and communicate changes with the physician and interdisciplinary team.
• Provide skilled nursing interventions and symptom management.
• Educate patients and caregivers regarding medications, treatment plans, comfort measures, and disease progression.
• Coordinate services provided by nursing, aides, social work, chaplaincy, and other disciplines.
• Participate in interdisciplinary team meetings and care planning.
• Advocate for patient and family goals and preferences.
• Maintain accurate, timely, and compliant clinical documentation.
• Provide support and education to caregivers throughout the hospice experience.

As a dedicated provider of hospice and palliative care, we are committed to delivering exceptional end-of-life care to patients and their families in the Central Ohio community. In this role, you will assess patients, develop individualized plans of care, and coordinate the delivery of hospice services to ensure the best possible quality of life for our patients.

Responsibilities:

• Assess patients and develop individualized plans of care
