Job summary

Buckeye Hospice and Palliative Care Services in Hilliard, OH is seeking a compassionate Registered Nurse Hospice Case Manager to support patients and families in the Central Ohio community.

You will assess patients, develop care plans, coordinate hospice services, monitor conditions, educate families, and collaborate with an interdisciplinary team to ensure comfort, dignity, and quality of life.

Qualifications

• Must hold an active Ohio RN license.
• Hospice, home health, or case management experience preferred.
• Strong assessment, organization, communication, and clinical decision-making skills.
• Ability to manage multiple patients and priorities independently.
• Commitment to compassionate, patient-centered care.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess patients and develop individualized plans of care
• Coordinate and oversee delivery of hospice services
• Monitor patient condition and communicate changes with the physician and interdisciplinary team
• Provide skilled nursing interventions and symptom management
• Educate patients and caregivers regarding medications, treatment plans, comfort measures, and disease progression
• Coordinate services provided by nursing, aides, social work, chaplaincy, and other disciplines
• Participate in interdisciplinary team meetings and care planning
• Advocate for patient and family goals and preferences
• Maintain accurate, timely, and compliant clinical documentation
• Provide support and education to caregivers throughout the hospice experience
