Frontier Hospice in Tulsa is seeking a full-time Hospice RN Case Manager (RNCM) to join our compassionate nursing team delivering hands-on hospice care and coordinating services for patients and families. The schedule is Monday–Friday, 8:00 am–4:30 pm, in the Tulsa territory with a focus on comfort and quality of life.

You’ll work with our interdisciplinary team, educate families, and support the patient journey with empathy and clinical excellence, strengthening care in the community.

Qualifikationen

• Must hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification when state requires.
• Valid driver's license and reliable transport.

Aufgaben

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct nursing care to ensure comfort and quality of life.
• Deliver nursing with heart and use proven principles to ease pain.
• Educate patients and families to develop personalized care plans.
• Share call duties as part of the team.
