Job summary

Bristol Hospice in New Albany, IN is seeking a dedicated RN Case Manager to guide patients and families through the hospice journey with compassion and clinical excellence.

You will develop care plans, monitor vital signs, administer medications, educate families, and collaborate with interdisciplinary teams in a full-time, Monday–Friday schedule. Competitive salary and comprehensive benefits available.

Qualifications

• Active RN license in good standing
• CPR certification current
• Strong written and verbal communication abilities
• Excellent observation and assessment skills
• Valid driver’s license with reliable transportation

Responsibilities

• Develop and implement individualized care plans based on patient history and current needs
• Deliver attentive, one-on-one support to patients and their families ensuring comfort and dignity
• Conduct thorough assessments of physical, emotional, and spiritual needs
• Monitor and document vital signs accurately (temp, pulse, resp, BP)
• Administer physician-ordered medications and treatments with care
• Educate patients and families on care techniques and symptom management
• Track changes in patient condition and respond promptly
• Maintain up-to-date clinical notes and collaborate with interdisciplinary teams
• Support additional duties as needed on a compassionate, mission-driven team
