Job summary
Lighthouse Hospice, Inc. is seeking a full-time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team serving Mercer, Burlington, Camden, Gloucester, and Salem counties. You will deliver direct hospice care, coordinate services, and guide patients and families through every step of their care journey.
We offer strong benefits, a supportive environment, and opportunities for growth. Hours are Mon–Fri, 8:00 am–4:30 pm, with on-call duties as needed.
Qualifications
- Hold current unencumbered RN license.
- Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
- Must be computer proficient, with EMR background; CPR may be required by state.
- Current driver's license, reliable transportation, and proof of insurance.
Responsibilities
- Empower Through Engagement: respond to clinical referrals with care and expertise.
- Collaborative Care Champion: work with Clinical Director to assess eligibility for hospice services.
- Nurturing Direct Care: deliver hands-on hospice nursing care focused on comfort and quality of life.
- Nursing with Heart: apply nursing skills to ease pain and bring solace to patients.
- Educator and Advocate: educate patients and families to participate in personalized care plans.
- Call of Compassion: participate in after-hours on-call duties.
