Job summary

Phoenix Home Care in Kansas City area is seeking a hospice RN Case Manager to provide compassionate, hands-on care to clients facing terminal diagnoses. You will work with an interdisciplinary team to develop and implement a patient/family-centered plan of care, guiding decisions to maintain quality of life.

The ideal candidate has an active Kansas or Compact RN license, hospice experience preferred (training provided), and a valid driver's license with auto insurance.

Qualifications

• Active Kansas or Compact RN license.
• Hospice experience preferred; training provided.
• Valid driver’s license and auto insurance.

Responsibilities

• Provide compassionate, hands-on hospice care.
• Support and educate patients and families.
• Complete assessments and ongoing evaluations.
