Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager to join our dedicated nursing team delivering compassionate end-of-life care across Beaufort, Jasper, Hampton, and Colleton counties. You will provide hands-on hospice nursing, coordinate services, and guide patients and families through every step of their hospice journey.

The role follows a Monday to Friday 8 am–4:30 pm schedule with a strong emphasis on education, advocacy, and collaboration with an interdisciplinary team.

Qualifications

• Active RN license
• 1 year med-surg/acute care experience; hospice experience a plus
• EMR proficiency and CPR certification as required by state
• Current driver's license, vehicle insurance, and reliable transportation

Responsibilities

• Respond to clinical referrals with care and expertise
• Collaborate with Clinical Director to assess eligibility for hospice services
• Provide direct nursing care as an RN Case Manager to ensure comfort and quality of life
• Deliver nursing care rooted in evidence-based practice to ease symptoms
• Educate and advocate for patients and families to participate in care plans
• Participate in on-call duties as part of the team
