Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager to join our compassionate nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm; Territory covers Beaufort, Jasper, Hampton and Colleton County. You will coordinate services, guide patients and families, and deliver hands-on hospice nursing care grounded in compassion and clinical excellence.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as RN in medical-surgical/acute care; hospice exp a plus.
• Proficient with EMR and computer programs; CPR certification as required.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Empower patients and families through education and care planning.
• Collaborate with Clinical Director to assess eligibility and plan care.
• Provide compassionate nursing support to ease pain and improve quality of life.
• Participate in on-call duties as part of the team.
